Participant Registration Agreement & Waiver

Please read carefully. By registering a child for Skate Beyond Limits, you acknowledge that you have read, understand, and agree to the terms below. If the participant is under 18, this agreement must be accepted by a parent or legal guardian.

Required Acknowledgments

I certify that I am the parent or legal guardian of the participant. *I have read and agree to the Participant Registration Agreement and Waiver. *I understand that ice skating involves inherent risks. *I authorize emergency medical treatment if necessary. *I consent to the collection and use of my information as described. *I understand that participation is voluntary, may be limited by program availability, and a parent / guardian should remain on-site. *
